Our Disease Management Programs work to improve the healthcare of members with diabetes or heart failure by discovering chronic conditions earlier. We work with members to slow down their diseases and decrease problems. Both our Heart Failure and Diabetes Management programs help members learn to better manage their conditions through support, information and medication management.

Program Overview
Participants have access to a Blue Cross nurse who works with their disease and the member's doctor, helps with individualized support, provides disease information, assists with coordination of care, serves as an advocate and coach, and helps members find qualified physicians.

  • We review past claims information to identify any members with diabetes or heart disease who may not have received important preventive services.
  • We review pharmacy claims information to identify members who can benefit from useful information about medications used to treat diabetes or heart failure and ways to improve medication management.
  • Eligible members are automatically enrolled; however, participation is voluntary.

Do I have a local representative?
All groups are assigned an account manager. Please contact the district office to inquire about your group's account manager.

What services are available to me online?

  • All employers are automatically enrolled in the Standard Option when they register for AccessBlue. The Enhanced Option, Member Maintenance, is a voluntary employer option you may select when you register. If you want to change your current level of functionality, call 1.800.229.6499.
    • Standard Option: Submit and review subsequent applications, including new hires.
    • Enhanced Option, Member Maintenance: Process coverage terminations; order replacement membership materials for individual employees or for your group; review, approve or deny submitted employee change requests that affect group, such as address changes, account corrections, adding or dropping dependents; verifying coverage qualifications; and reporting other coverage.
  • You can also pay your group bill online through your bank's system.
  • At our Forms Library, you and can download the forms you need most often.
  • Our Online Provider Directory, which is updated daily, gives you and your employees access to thousands of doctors and hospitals in the Blue Cross network.

How do I register my group for AccessBlue?
As an employer, you can register your group online. AccessBlue is a handy administrative tool you can use to review changes submitted by your employees, and to get new employees enrolled in your plan.

Why can't I change my employees' address or other information in AccessBlue?
At this time only members have access to change their personal information; however, group administrators have access to review their group member changes.

Can I view and pay my bill online?
You can pay your group bill online through your bank's system. Be sure to include your current contract number in the account field stored under payee Blue Cross.
